2011 School Supply Giveaway/Soles 4 School

2011 School Supply Giveaway/Soles 4 School will be held at the Delaware County Fairgrounds, July 30, 9 a.m. – 1 p.m.

Three registration sites will open @ 7:00 am
Doors open @ 9:00am – 1:00pm

The Memorial Building will be the site for the School Supply Giveaway, hosted by Hearts & Hands, with back packs being provided filled with various, grade appropriate school supplies.

Soles 4 School tennis shoe giveaway, hosted by Glad Tidings Church, will be held in the Community Building with shoes being provided for students in all grades.

REQUIREMENTS:
Parent/legal guardian must accompany all children through 8^th grade;
Provide photo ID and school ID
•Immunizations for those without insurance
Childhood immunizations for ages K-12 will be available on July 30^th from 9:00am to 1:00pm at the Heartland Building.

In order to receive immunizations, you must provide for your student(s) - - NO EXCEPTIONS:
     »Current and up-to-date immunization record
     »A parent or legal guardian attend with the child
     »Written consent of parent or legal guardian if another adult is bringing a child
     »Knowledge of child’s allergies and current state of health

You will also be asked to fill out paperwork to see if you qualify for immunizations free of charge. Students who are un-insured or under-insured will be able to receive shots and immunizations free of charge. You will also be asked to fill out paperwork to help us know what immunizations your child can/needs to receive.
